Rates
$125  per 50 minute session

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. I am an approved provider on most major insurances.  Please check your coverage carefully by asking the following questions:

• Is Michelle Hamilton an in-network or out-of-network provider?
• Do I have Behavioral or Mental Health insurance benefits?
• Do I need an Authorization number?
• What is my deductible and has it been met?
• Do I have a copayment or coinsurance?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Reduced Fee
Reduced fee services are available on a limited basis.

Payment
Cash, check and all major credit cards accepted for payment.  Your portion of the payment is expected at the time of service.

Cancellation Policy

If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay a fee for the session.  INSURANCE DOES NOT COVER THIS FEE.  If you do not show for 2 consecutive sessions, payment will need to be paid in full at the time of scheduling any future appointments.
